		<description>At one company I know, if you want access to more than just the basic health plan, you have to sign up for the wellness program and reach a certain threshold.  You can sign up for various health initiatives and/or instruction to reach the minimum threshold.  Also if you exceed the minimum threshold by certain amounts, you are then eligible for discounts against your insurance premium (the more you exceed the threshold, the higher your discount - to a certain point).  Also, some companies use Matria Healthcare (http://www.matria.com) to help manage wellness programs.
